NIBP under-inflation
Fault phenomenon: blood pressure measurement times "cuff too loose" or cuff leakage, inflation pressure is always not filled up (below 150mmHg), can not be measured.
Check method.
(1) There may be a real air leak, such as the cuff, air guide tube and various joints, through the "air leak detection" can be determined.
(2) The patient mode selection is not correct, if you use the adult cuff but the monitor patient type is used for newborns, there may be this alarm.
Exclusion method: Replace the blood pressure cuff with a good quality or choose the right type of cuff.
NIBP measurement value is not accurate
Fault phenomenon: the measured blood pressure value deviation is too large.
Checking method: Check whether the blood pressure cuff is leaking, whether the connection with the blood pressure tube interface is leaking, or whether it is brought about by a difference under subjective judgment with the auscultation method?
Exclusion method: Use the NIBP calibration function. This is the only available criterion to verify the correct calibration value of the NIBP module at the user's site, and the NIBP is factory tested for a pressure standard deviation of 8 mmHg or less. If this is exceeded, the blood pressure module needs to be replaced.
Transcutaneous Oxygen Saturation Monitoring
1. The infrared probe of the transcutaneous oxygen saturation monitor is fixed to the patient's fingertip, and the percentage of oxygenated hemoglobin to hemoglobin is monitored to the pulsation of the small artery at the patient's fingertip.
2、Cautions.
①The probe should be fixed when used and the patient should be kept as quiet as possible to avoid alarm and non-display of results.
②The accuracy of the results can be affected when the peripheral circulation is poorly perfused such as severe hypotension and shock.
No blood oxygen value or intermittent blood oxygen value
Failure phenomenon: When measuring human oxygen saturation, the oxygen value is intermittent.
Check method.
①When doing long-term monitoring and surgery, whether the patient's arm is moving around, causing the blood oxygen value to be intermittent.
②Check whether the oxygen extension cord is bad.
Solution: Keep the patient as stable as possible. Once the oxygen value is lost due to hand movement, it can be considered normal. If the oxygen extension cord is broken, replace it with a new one.
Blood pressure, oxygen, respiratory monitoring, common problems
1、Inaccurate blood pressure measurement
2、Inaccurate blood oxygen measurement, the screen display probe off, etc.
3、Abnormal display of respiratory parameters, false alarm, etc.
Treatment
Maintain correct body position, correct pressure measurement site and proper cuff tightness, quiet and less interference during each blood pressure measurement. Choose neonatal or adult gear according to the age, and release the residual gas in the cuff when taking each measurement. The difference between the blood pressure reading and that measured manually is more than 4 mmHg.
Fix the probe as much as possible to keep the patient quiet, if the screen shows that the sensor is off, then judge the sensor position, the sensor is in place and good performance, then pay attention to whether the connection is normal, or whether the liquid has spilled into the sensor connector. After turning on the machine, test whether the blood oxygen probe is working properly, make sure that the probe emits a darker red light or brighter red light and flickering after the self-test.
Cardiac monitoring common alarm processing process
I. When heart rate abnormal alarm
1、First of all, check whether the electrode placement is correct, whether the electrode is in good contact with the skin, whether the electrode has fallen off, etc., to exclude interference factors
2、Check whether the alarm value is set in accordance with the condition to avoid missing and invalid alarm.
3、Monitor the heart rate with stethoscope to verify and confirm abnormal heart rate
4、Immediately notify the physician on duty and make preparations for resuscitation
5, timely and accurate implementation of medical advice, and actively cooperate with the physician for resuscitation treatment
6、Closely observe the reaction after medication and monitor the change of the patient's heart rate
Second, when the abnormal heart rhythm alarm
1、Nurses master the ECG changes of common arrhythmias, especially malignant arrhythmias
2、First check whether the electrode is correctly placed, whether the electrode is in good contact with the skin, whether the electrode is dislodged, etc., and exclude interfering factors
3、Immediately notify the physician on duty and make preparations for resuscitation
4、Timely and accurately execute medical prescriptions, and actively cooperate with physicians to carry out rescue treatment
5、Closely observe the reaction after medication and monitor the change of the patient's heart rhythm
Third, when the blood pressure is abnormal alarm
1、First of all, check whether the cuff position is appropriate, whether the tightness is suitable, whether there is air leakage, etc.
2、Check whether the setting of the alarm value is in line with the condition to avoid missed and invalid alarms.
3、Repeat the measurement to confirm abnormal blood pressure
4、Immediately notify the physician on duty and make preparations for resuscitation
5, timely and accurate implementation of medical advice, and actively cooperate with the physician for resuscitation treatment
6、Closely observe the reaction after medication and monitor the change of the patient's blood pressure.
Four, oxygen saturation abnormal alarm
1、First of all, check whether the finger cover is correctly placed and whether there are any interference factors such as dislodgement
2、Monitor the patient's circulation and respiration to confirm the presence of abnormal oxygen saturation
3、Adjust oxygen flow, remove respiratory secretions and keep the airway open
4、If not relieved, immediately notify the physician, make preparations for resuscitation and correct hypoxemia
5、Closely observe changes in the condition and monitor the improvement of blood saturation
